Appointments: How to Proceed?
Through your family doctor or specialist
Systemic diseases that involve your vision are well known by your family doctor and/or specialist. These must contact us directly and make an appointment by phone, fax or online. We offer a wide variety of ophthalmological consultations in Montreal, tailored to your needs.
Through your optometrist
Your optometrist may also have noticed abnormalities during your routine exam and may ask us for a consultation. Your optometrist can communicate directly with us and make an appointment by phone, fax or online.
It is mandatory to have your healthcare provider fill out the required fields on the ophthalmology consultation request and send it to us by fax at 514-593-3937.
The medical professional’s signature and practice number must appear on the request. The request must also contain your name, your phone number as well as your date of birth. One of our secretaries will contact as soon as possible afterwards.

In preparation of your consultation
When you come in for you consultation, please bring your consultation request, a list of taken medications and your glasses (if applicable). Make sure your health insurance card is valid. Be aware that some administrative fees to the examination can be applied, we will notify you in advance.
If possible, come accompanied.
